- Fill up the online registration form available on your appointment confirmation by WhatsApp
- Bring identity card (KTP/KITAS/Passport)
- Fill up pre-assessment form provided by the hospital
Radiology (X-Rays / CT Scan)
- Not wearing any jewelry or metal accessories
- Informed our staff if you have history of orthopaedic surgery with plate and/or screw fixation, heart stent, dentures.
- For women, please inform our staff in case of any pregnancy
Laboratory
- Fasting for 10-12 hours before blood sample collection, drinking plain water is permissible
- Avoid consumption of any kinds of supplements/vitamins, alcohol and smoking
Physical and Eye Examination
- Bring or wear your glasses
- Please inform the nurse if you are currently under any medication
- Have enough rest, avoid heavy physical activities or working overtime before Medical Check Up
Abdominal Ultrasound
- Fasting for 4-6 hours before check up
- Bladder has to be full (please make sure to drink a lot of water and refrain from urinating)
